Tragedy at Sea: Lessons Learned from the Brufjell Incident

Brufjell has become a focal point in maritime safety discussions following a recent report by the Dutch Safety Board, which highlighted a series of operational failures that ultimately led to a fatal accident aboard the ship, 'Damsterdijk', in October 2019. The investigation revealed that poor communication among crew members and the fatigue of key personnel were significant factors contributing to the tragedy, which resulted in the untimely death of a 57-year-old Indonesian crew member. The accident occurred during mooring operations at the port of Ipswich, where the victim was handling mooring lines that became entangled in the ship's propeller, causing severe injuries. Despite being transported to the hospital, he succumbed to his injuries later that day.

The report underscored the importance of well-defined communication protocols during critical maneuvers, such as the mooring and unmooring of vessels. The review pointed out that the responsible sailor on the aft deck—a vital position that requires constant vigilance—was unable to maintain situational awareness because he was tasked with multiple responsibilities at once. This lack of oversight led to a dangerous situation where the victim was left without proper guidance and communication devices like a walkie-talkie. Additionally, it was noted that a 'clean working deck' was not maintained, allowing loose lines to run at high speeds into potentially hazardous areas.

The tragic incident was not the first time the 'Damsterdijk' faced challenges at sea; it also made headlines in 2012 when it suffered an engine failure while navigating the Kiel Canal. Subsequently, it required the assistance of two German tugs to get safely to the port of Rendsburg.
