The Amoy-Dream: Crew Feedback and Vessel Data

The United Kingdom's Marine Accident Investigation Branch (MAIB) has published a detailed report regarding a fatal accident involving the 'Mona Manx', which was operating under the name 'Amoy Dream' during the incident. On August 26, 2021, while the vessel was maneuvering to berth at Puerto Ventanas, a tragic event unfolded leading to the death of the second officer (2/O). As the vessel was docking, the 2/O was found leaning over the side when he was unexpectedly struck by a recoiling mooring line that had become trapped between the ship and the quay, likely snagged on a fender. This recoil resulted from the ship's astern maneuvering under its own power. At the critical moment, at 08:36:11, the 2/O approached the front end of the accommodation ladder to get a better view of the mooring line while using a very high frequency (VHF) radio. Just seconds later, the line went tight and then slack. By 08:36:23, as the line gained tension again, a linesman on the quay moved closer to it when a loud bang suddenly pierced the air as the spring line released and shot upward. Despite the linesman’s quick retreat, the recoiling line struck the 2/O beneath his chin, propelling him backward into the accommodation ladder above him. The impact sent his safety helmet flying off and caused his VHF radio to fall into the water as he collapsed onto the deck, bleeding significantly. An Able Seaman (AB3), who was searching for him, discovered the 2/O and immediately alerted the captain using another crew member's VHF radio about the severe injury. Rescue operations were initiated promptly, with a medical team arriving on the scene shortly after. Tragically, the 2/O was pronounced dead shortly after 08:50. The MAIB report highlights several safety concerns, including: - The 2/O's decision to position himself in a hazardous area to gain a clearer view of the mooring line - The inappropriate maneuvering method using engine power to reposition while docked - Insufficient guidelines within the Code of Safe Working Practices for Merchant Seafarers (COSWP) regarding mooring line hazards - Incomplete information during the master/pilot exchange (MPX) prior to the operation - Lack of crew briefing about the maneuver's risks - Radio traffic congestion limiting effective communication among crew members In light of these findings, the MAIB has recommended that Puerto Ventanas S.A review and enhance the protocols available to vessel masters and pilots prior to docking, emphasizing that engine use is prohibited during mooring and outlining the risks related to line entrapment.
