On July 23, 2026, the Transport Accident Investigation Commission (TAIC) unveiled a pivotal report detailing a hazardous incident onboard the Antarctic Discovery that occurred on February 21, 2025. During routine maintenance while the vessel was dry-docked in Lyttelton, workers faced a critical emergency when a pump inadvertently released decomposing fish offal into a machinery space, resulting in the buildup of toxic gas and leading to the hospitalization of four crew members. The incident unfolded as two apprentice fitters descended a ladder to access the bow thruster machinery space, where they were tasked with repairing pipe components situated next to a tank containing a mixture of decaying fish offal and seawater. A major risk materialized when one of the fitters mistakenly activated the unlabelled start button on the offal pump, triggering a cascade of hazardous contents from the holding tank. Toxic hydrogen sulphide gas swiftly accumulated in the confined area, forcing the fitters to navigate through the hazardous deluge and climb the ladder to reach safety. In an attempt to assess the situation, the vessel superintendent also ventured below but was quickly incapacitated by the gas. The chief engineer and second engineer followed, both collapsing and requiring rescue with breathing apparatus from other crew members. Fortunately, all four workers were eventually hospitalized, recovering from the incident. TAIC's Acting Chief Investigator, Tahlia Fisher, reported that while the accidental activation of the offal pump was deemed the immediate cause of the accident, investigators identified four broader safety concerns affecting not only maritime operations but various industries. Key recommendations included ensuring that all machinery, especially nearby systems, be rendered safe prior to commencing work, and treating any space with confined-space characteristics with the appropriate safety precautions. Furthermore, the report highlighted the necessity of clear coordination among multiple organizations operating in shared workplaces to ensure collective safety accountability. It emphasized that safety-critical modifications require stringent oversight, including formal approval and detailed documentation. Although Australian Longline Pty.
