MCIB Report Highlights Safety Shortcomings in Arklow Clan Accident

In a significant revelation, the Irish Marine Casualty Investigation Board (MCIB) has released its findings concerning an incident on the vessel "Arklow Clan," which occurred on August 12, 2021. The report details how the Second Officer suffered serious injuries after losing his balance while lowering handrails, resulting in a fall of approximately 3.6 meters from the walkway to the quay below. Due to the severity of the impact, the officer required extensive hospitalization, multiple surgical procedures, and a lengthy rehabilitation process. The investigation pinpointed a critical failure to adhere to safe work practices for activities conducted at elevated heights. Key deficiencies identified included a lack of proper risk assessment, the absence of a permit to work, and neglect in conducting essential “toolbox talks” prior to commencing the task. These safety gaps were promptly addressed by Arklow Shipping following the incident. While it was determined that the Second Officer was not excessively fatigued, it was noted that the challenges of working night shifts, particularly from midnight to 4 a.m., may have contributed to a momentary lapse in concentration, coinciding with the accident. In light of these findings, it is recommended that the Minister for Transport issue a Marine Notice to remind vessel operators and crews of the inherent risks associated with working at heights. Emphasizing compliance with existing regulations and recommendations is vital to mitigate future incidents. This includes ensuring thorough risk assessments, obtaining necessary permits to work, conducting appropriate safety briefings, and providing adequate personal protective equipment (PPE). Moreover, crew members should receive training on proper PPE usage, with regular inspections and maintenance scheduled in accordance with the International Safety Management (ISM) Code, applicable to both passenger ships and cargo vessels exceeding 500 Gross Tonnes.
