Berge Rederi AS Enhances Safety Protocols Following Overboard Incident

Sep 29, 2022 11:51

Following a tragic incident involving a cadet who fell overboard from the MV Titran on March 6, 2022, Berge Rederi AS has taken significant steps to bolster safety measures on their vessels. The unfortunate event occurred during the preparation for a fishmeal delivery at the facility in Halsa, Meløy. As the deck crew was removing protective wedges on the hatch deck, a cadet inadvertently fell into the water while transitioning from the closing deck to the main deck after working on the excavator. It wasn't until approximately 20 minutes later that the crew discovered the cadet missing, as no one had witnessed the accident, according to a report from the Norwegian Maritime Accident Investigation Board. Once the cadet's absence was noted, the ship's captain promptly alerted rescue services and turned the vessel around to search for the missing crew member. However, the initial search efforts were hampered by uncertainty regarding the timing of the fall. It took around two hours to ascertain the correct timeframe using surveillance footage, which allowed the search to be redirected southward, closer to the actual site of the accident. While some personal items were recovered, the cadet was sadly not found. The Accident Investigation Board has emphasized the importance of clear communication among crew members regarding the timing of overboard incidents to assist rescue operations effectively. In response to the accident, Berge Rederi AS has collaborated with its crews to devise practical safety enhancements, including: - Mandatory life jacket use for all personnel engaged in external operations and while on the hatch deck. - Consideration of personal warning devices to alert for any falls overboard, although these have yet to be implemented. - Clearly marked "unsafe zones" on the hatch deck where fall protection harnesses will be required. - Plans to install a safety cable along the hatch deck for crew connection. - Proposals to elevate railings on the MV Titran and similar vessels to enhance protection. - Updated guidelines defining approved access routes to and from hatch decks. - Regular safety meetings to reinforce situational awareness from training exercises. - Training courses aimed at improving onboard safety consciousness among crew members. Notably, the Norwegian Maritime Accident Investigation Board has opted not to issue formal safety recommendations, indicating that the investigation did not uncover grounds for further safety measures to enhance maritime safety at this time.

Freighter Incident: Crew Member Overboard in Lurøy

Mar 09, 2022 10:51

On March 6, 2022, at approximately 11:30 a.m., a distressing situation unfolded aboard the freighter 'Titran' when a crew member was reported missing and presumed overboard in Lurøy, Norway. The incident occurred in the treacherous waters between Kvarøy and Tonnes, where the individual, tragically without a life jacket or survival suit, fell into the sea. Crew members were engaged in deck work when they realized one of their own was unaccounted for. Despite attempts to reach him via radio, their efforts proved futile. Emergency response was swift, with a rescue helicopter and multiple boats deployed to search for the missing man. By around 2 p.m., the search teams discovered items in the water, prompting them to adjust their search area slightly further south. Fortunately, conditions for the search were optimal, allowing teams to perform comprehensive sweeps both on land and at sea. However, as night descended, the search was called off without any sign of the crew member, and officials changed the mission’s status to recovery. Rescue operations resumed the following morning, March 7, as efforts continued to locate the presumed deceased individual.
