Critical Maintenance Oversights Uncovered in RRS 'Sir David Attenborough' Lifeboat Drill Incident

Nov 06, 2023 12:57

A recent investigation by the UK Marine Accident Investigation Branch (MAIB) into a lifeboat drill incident aboard the RRS 'Sir David Attenborough' has brought to light alarming maintenance deficiencies that contributed to the accident. The report, released this week, details the events of March 4, 2021, when a critical interlock device associated with the lifeboat davit failed during a routine drill near the Isle of Mull in Scotland. This failure led to the port lifeboat plummeting into the water with three crew members inside. After a shocking drop onto the ship's deck, the lifeboat was subsequently dragged overboard by the moving davit arms and detached from its hooks, resulting in minor injuries to those onboard. The 'Sir David Attenborough', delivered just months prior by Cammell Laird Limited, was undergoing its first lifeboat drill at sea. The MAIB's findings indicated that the significant corrosion that caused the interlock device failure stemmed from inadequate maintenance of the less than one-year-old vessel. Notably, essential inspections and planned maintenance on the davit had not been conducted since its installation in November 2019. Furthermore, the investigation revealed that the installation of the lifeboat davits was incomplete and non-compliant with both manufacturer guidelines and international regulations. Following the incident, the research vessel returned to the builder’s yard for necessary repairs, including the proper installation of the davit system. The report highlighted several safety lapses, notably the incompleteness of safety equipment surveys that failed to catch the non-compliance with SOLAS standards. Additional issues included insufficient training for the crew in operating the davit and the suspension of critical equipment maintenance by the ship's operator. In light of these findings, the MAIB has issued two key recommendations: first, to encourage the Maritime and Coastguard Agency (MCA) to reevaluate its procedures for delegating Safety Equipment Surveys to Recognised Organisations to ensure strict adherence to SOLAS regulations; and second, to assess its delegation policies focused on initial safety equipment surveys for newly constructed vessels or those joining the UK fleet.
