Fatal Accident at Keppel Shipyard: Worker Killed by Overweight Rudder
In a tragic incident at Keppel Shipyard in Singapore, 36-year-old employee Mandeep Kumar lost his life due to multiple injuries sustained from a falling rudder of the 'City of Shanghai' in March 2020. During the coroner's inquiry, it was revealed that the rudder weighed 59 tonnes, significantly exceeding the safe capacity of the faulty shackle that was designed to hold 35 tonnes, resulting in a 24-tonne overload. While Mandeep was conducting repairs on the 'City of Shanghai', the heavy rudder dislodged unexpectedly and struck him. In a desperate attempt to maintain his balance, he clung to a section of the ship located 7.3 meters above ground. Unfortunately, he was knocked off balance when the rudder crashed into him, severing his right arm and causing him to enter cardiac arrest. He was pronounced dead at the scene. Another worker, Mr. Miah Md Masum, was also in the vicinity and suffered minor injuries. Investigations by the Ministry of Manpower uncovered that senior foreman Inbaraj Vellu had neglected to account for the additional weight of components connected to the rudder, which he estimated at 32 tonnes without proper verification of his calculations. Furthermore, Keppel Shipyard determined that Mandeep was improperly positioned on the rudder instead of using the scaffolding nearby or a hydraulic crane for safety. He also failed to secure his safety harness to an anchor point. On March 19, 2021, State Coroner Marvin Bay classified the incident as a "tragic industrial misadventure," emphasizing the imperative for heavy industry stakeholders to comprehend and mitigate the inherent risks. The coroner called for rigorous risk assessments and measures to validate the safety and operational fitness of all equipment to prevent potential harm to workers and avoid scenarios where they could be vulnerable to machinery failures.
