Investigation Reveals Cardiac Issues Led to Fatal Incident on Sage Amazon

Aug 07, 2019 12:19

The Transportation Safety Board (TSB) has released its investigation report concerning the tragic death of the master of the 'Sage Amazon' off the coast of Port-Daniel-Gascons in March 2018. The report indicates that the fatal incident was caused by a cardiac ailment. Despite a known history of health issues, including a prior heart condition, the captain had been deemed fit for duty. On March 17, 2018, while accessing the cargo hold via a ladder, the 54-year-old Ukrainian experienced a heart attack and fell, suffering severe head injuries upon impact with the deck. Initial medical response focused on addressing his head injuries before cardiovascular resuscitation efforts were initiated. The TSB's findings highlight alarming discrepancies in the assessment of the captain's health, as the maritime industry standard for fitness to work at sea should have taken his previous cardiac issues into greater account. An independent review of his medical history by the TSB confirmed that the captain had a documented history of cardiac problems, which should have influenced his fitness for maritime work. This report stresses the importance of rigorous health evaluations to ensure the safety of personnel operating at sea.

TSB Launches Investigation into Fatal Incident on Bulk Carrier Saga Amazon

Mar 18, 2018 18:22

The Transportation Safety Board of Canada (TSB) has dispatched a specialized team to Quebec to probe the tragic death of a captain aboard the bulk carrier 'Saga Amazon.' This unfortunate incident occurred on March 17, 2018, as the vessel was anchored offshore, waiting to load a cement shipment from McInnis Cement. The ship was positioned approximately four miles off the coast of Port-Daniel-Gascons, on the southern edge of the Gaspé Peninsula at the time. The 54-year-old Ukrainian captain fell three meters into the hold of the ship during the loading operation. Emergency response teams moved quickly, with two rescue specialists equipped with a defibrillator arriving by fast boat. Despite their urgent attempts to revive him, the captain could not be saved. In the rescue efforts, the Canadian Coast Guard icebreaker CCGS 'Terry Fox,' a National Defense helicopter, and a local water rescue team from the RCM Rocher-Percé were all activated after the Coast Guard received the emergency call around 7:30 a.m. The Rocher-Percé rescue team was deployed at 9:15 a.m., with the 'Terry Fox' arriving first on the scene, where its medical crew made the initial attempts at resuscitation. Unfortunately, despite the arrival of additional medical support from the helicopter and the water rescue team, the captain was pronounced deceased. Following the incident, Transport Canada mandated that the 'Saga Amazon' remain docked until a new licensed captain could take over operations. A Romanian captain is anticipated to arrive on the afternoon of March 20 to assume command of the vessel.
