On October 18, 2017, at approximately 5:30 PM, the National Sea Rescue Institute (NSRI) received an urgent alert from the Maritime Rescue Coordination Centre (MRCC) regarding a crew member aboard the bulk carrier 'Grand Amanda.' The crew member sustained severe injuries, including fractures to both legs and thighs, while the vessel was situated roughly 189 miles off the South African East Coast, prompting a course adjustment towards East London. A duty doctor from Government Health EMS provided crucial medical guidance to the crew to assist in treating the injured individual amidst challenging sea conditions. Unfortunately, the considerable distance from shore made a helicopter medevac unfeasible. Instead, the ship was directed to the nearest port for further assistance. As the night progressed, efforts were made to arrange a medevac for the following morning. The Transnet National Ports Authority (TNPA) deployed a pilot boat to transport paramedics to the 'Grand Amanda' as it neared East London. However, adverse weather conditions, with rough seas and gusts of up to 50 knots from the east, hindered the paramedics' boarding. Throughout the night, various agencies, including NSRI Durban, NSRI Port Elizabeth, NSRI ASR, the TNPA Port Helicopter, and the South African Air Force's 15 Squadron, remained on high alert, preparing for a possible helicopter rescue if the weather improved. During this time, medical supplies were successfully transferred aboard the vessel. In a turn of events on the early hours of October 19 at around 5 AM, NSRI East London was poised to attempt boarding the 'Grand Amanda' at sea when it was tragically confirmed that the injured crew member had succumbed to his injuries and passed away. This incident highlights the perils faced by those working in maritime environments and the challenges rescue teams encounter under daunting weather conditions.
