False Alarm Leads to Grounding Incident of 'Heemskerkgracht' in Kahnawake
The Transportation Safety Board of Canada (TSB) has released a comprehensive report detailing the grounding of the 'Heemskerkgracht' on August 22, 2024. This incident occurred in the South Shore Canal of the St. Lawrence Seaway near Kahnawake, Quebec, following an unexpected loss of propulsion. According to the TSB, the vessel's main engine halted operations due to a false alarm triggered by the oil mist detector (OMD). While the ship was anchored and awaiting assistance, it eventually required refloating and was towed to the Port of Côte-Sainte-Catherine, where it sustained only minor hull damage. Oil mist detectors, designed to prevent serious engine failures, automatically shut down the engine upon detecting oil mist. However, a limitation of these devices is their inability to distinguish between oil mist, water vapor, or smoke, leading to potential false alarms. In this case, improper maintenance significantly contributed to the malfunction. During regular servicing, the OMD measuring head was replaced with a different model that depended on an external power source for its heater. In contrast, the original model was self-powered. Consequently, the heater remained unpowered, making the measuring head susceptible to false alarms from water vapor accumulation. The TSB investigation concluded that the buildup of water vapor caused the false alarm, resulting in the automatic shutdown of the engine and the vessel's grounding. Notably, after the incident, the vessel operator acted swiftly to replace the OMD measuring head with a unit identical to the original model. The new device was installed following the manufacturer's guidelines and underwent proper calibration and testing. This case highlights the critical need for strict oversight when making equipment modifications, thorough documentation processes, and routine verification to ensure that safety systems are in optimal working order. The TSB's findings serve as an important reminder that even well-designed safety solutions can pose new risks if they are not managed effectively.
