ATSB Report Reveals Lookout Failures in Moreton Bay Collision

In a comprehensive investigation, the Australian Transport Safety Bureau (ATSB) has released findings regarding the July 6, 2014 collision involving the 'Kota Wajar' container ship and the yacht 'Blazing Keel'. According to the report, both vessels failed to keep a proper lookout, resulting in a collision at 04:19 a.m. in clear visibility conditions within Moreton Bay. The Kota Wajar was navigating southbound in the shipping channel while the Blazing Keel crossed the channel in a southwest direction. Although the yacht sustained significant damage, it managed to retain its watertight integrity, with no injuries reported among the two individuals on board, who were able to safely return to their marina. The ATSB investigation emphasized that no crew members on either the Kota Wajar or the Blazing Keel detected the other's presence prior to the collision. The report highlights a clear failure to adhere to the COLREGS regulations regarding watchkeeping. Notable shortcomings were found in the safety management practices aboard the Kota Wajar, where a dedicated lookout was not deployed and radar usage was improperly managed. Contributing factors included the high workload aboard the ship's bridge and the distractions of local background lights. Additionally, the crew of the Blazing Keel kept an ineffective visual lookout, compounded by the yacht’s radar being inoperable for the past 18 months. The investigation also points out deficiencies in the passage plan and master-pilot exchange protocols of Brisbane Marine Pilots, which fell short in ensuring that the ship's bridge team received essential information on local traffic and small craft attention points. In response to these findings, Pacific International Lines, the managers of Kota Wajar, have committed to enhancing safety protocols related to lookout postings. Brisbane Marine Pilots is also taking action by revising its standard passage plans to clarify lookout responsibilities and emphasizing communication regarding small vessel interactions through the master-pilot-bridge team exchange. As a proactive measure, they are reviewing pre-arrival information for masters to further address the risks of small vessel interactions. To address the ongoing safety challenges associated with effective lookout maintenance in Australian waters, the ATSB has issued a safety advisory notice to all vessel masters, owners, operators, and skippers.
