A recent investigation by the Marine Accident Investigation Branch (MAIB) has revealed that a catastrophic engine failure was responsible for a fire onboard the 'Kommandor Susan' during sea trials in the Firth of Forth on January 25, 2025. The incident began shortly after the vessel departed from Leith when it experienced a complete power blackout, resulting in a loss of propulsion and causing the ship to drift. Crew members reported a loud bang just moments before the fire alarm sounded, indicating a critical malfunction. Thankfully, after extinguishing the fire, the crew managed to restore partial power, and with assistance from two towboats, the 'Kommandor Susan' safely returned to port. Remarkably, there were no injuries reported among the crew during this unsettling event. The MAIB highlighted significant safety concerns in its findings, particularly the use of 'non-genuine' engine components during a major overhaul conducted in 2019, which may have contributed to the failure. Following the fire, the engine was successfully rebuilt using original manufacturer's parts. While the MAIB did not issue any formal recommendations following its investigation, it did reach out to the vessel’s previous owner, stressing the importance of closely monitoring all essential maintenance operations to prevent similar occurrences in the future.

On June 24, 2025, the vessel 'Kommandor Susan' found itself detained in Delfzijl due to the identification of six deficiencies. Among these, two were classified as serious, warranting the vessel's detention. The critical issues noted were: the expiration of the Special Purpose Ship Safety certificate, and the non-compliance regarding the ISM (International Safety Management) certificate.
