Complete Investigation Report on 'Mazarine' Grounding Released
On July 16, 2024, the Marine Safety Investigation Unit of Transport Malta unveiled a detailed report regarding the unfortunate incident involving the 'Mazarine,' which ran aground on Wolf Rock near Land’s End, UK, on July 10, 2023. The vessel experienced significant structural damage to its underwater hull, suffering multiple perforations and deformations, primarily on its port side. The report pointed to a critical failure in an elbow within the main engine's lubricating oil system as the catalyst for the incident, leading to a considerable loss of lubricating oil and a subsequent drop in oil pressure. Under extreme pressure to address the situation, the engine-room crew made two urgent attempts to rectify the lubricating oil system. The investigation brought to light several key factors that contributed to the grounding: fatigue in the elbow pipe due to alternating stresses potentially linked to high vibration levels or bending forces from a degrading rubber bellow; a lack of propulsion from the main engine as a direct result of diminished lubrication oil pressure; and a failed repair attempt due to inadequate conditions for welding and resin repairs. Compounding the situation, the crew was unable to deploy the anchor effectively due to the water depth and drift rate, drastically reducing their chances of stabilizing the vessel. No spare pipe was available aboard, and repair attempts were further hampered by intense working conditions and inadequate surface preparation. Fortunately, the crew took timely actions regarding visual signals and indications to denote the vessel was Not Under Command (NUC), successfully alerting nearby vessels and the Falmouth Coastguard. Actions taken immediately before, during, and after grounding assured the safety of passengers and crew while effectively managing and assessing the damage. Although the captain considered alternative anchoring tactics, he ultimately chose not to proceed due to concerns about the risk of twisted cables. Additionally, a malfunction with the Voyage Data Recorder (VDR) hindered the retrieval of operational data. In light of the findings, the report urges Anglo-Eastern Ship Management (NL) BV to investigate vibration levels in the elbow pipe and implement corrective measures if necessary. It also recommends that MAK issue a service bulletin for operators to conduct non-destructive testing (NDT) on similar piping to prevent future incidents. On April 17, 2019, the freighter 'Mazarine', while traveling from Rotterdam to Dublin, faced a significant disruption when a leak in the main engine's cooling water system rendered it disabled.
