On March 29, 2022, the National Transportation Safety Board (NTSB) released a comprehensive report detailing the underlying issues that led to a significant incident involving the tanker 'Riverside'. The investigation highlighted a critical maintenance challenge linked to the propulsion systems of single-screw merchant vessels that can result in a loss of maneuverability during critical navigation phases. This unfortunate breakdown occurred on March 15, 2021, as the 'Riverside' was departing the port of Corpus Christi, Texas, at approximately 1:02 p.m. During the incident, the 'Riverside', which was fully loaded with 700,000 barrels of crude oil, experienced a propulsion failure after pilots boarded at 10:18 a.m. The vessel had just exited Corpus Christi's inner harbor, with its escort tugs having departed following a speed increase to 10.5 knots. To facilitate the safe departure of the 'Nordic Aquarius', another tanker preparing to leave the pier at the Moda Ingleside Energy Center, the pilot of the 'Riverside' opted to reduce speed, which led to progressively halting the engines by 12:55 p.m. As the ship's heading began to veer to port shortly thereafter, attempts to regain control through helm orders were unsuccessful. At 12:58 p.m., recognising the urgency of the situation, the pilot instructed to engage the engine at dead slow ahead to restore steerage. Unfortunately, the engine failed to restart from both the bridge and engine room control panels. As the 'Riverside' drifted at around six knots towards the Moda pier, nearby tug 'Honor' responded to the pilot’s request for assistance. With limited time to act, 'Honor' managed to nudge the tanker slightly off its collision course, but the 'Riverside' still struck the pier at a speed of five knots, resulting in an estimated $7 million in dock damage. Despite the impact, 'Honor' and three additional nearby tugs were able to halt the vessel’s momentum within another 1,600 feet. This report serves as a critical reminder of the importance of operational maintenance and emergency readiness for maritime vessels.
