USS Truxtun Commander Relieved After Collision with USNS Supply
In a significant leadership shakeup within the U.S. Navy, Cmdr. James Koffi has been removed from his command of the USS Truxtun (DDG-103) following a maritime incident with the USNS Supply (T-AOE-6) on February 22. The decision to relieve Koffi stemmed from a 'loss of confidence' regarding his command capabilities, as stated by Rear Adm. Carlos Sardiello, the head of Naval Forces Southern Command. In the wake of this incident, Cmdr. Taylor Auclair, who previously served with the U.S. Fleet Forces Command, has stepped in as the interim commander of the Arleigh Burke-class destroyer. Cmdr. Koffi has been reassigned to Commander, Naval Surface Group Middle Atlantic. The USS Truxtun was just weeks into its Caribbean deployment after departing Naval Station Norfolk when the collision occurred during a replenishment at sea, as the ship unexpectedly veered to starboard, striking the oil supply vessel. The Navy is continuing its investigation into the circumstances surrounding the crash.
USS Truxtun and USNS Supply Collide in Caribbean Sea During Replenishment Operation
On February 10, 2026, the USS Truxtun (DDG-103, MMSI: 303882000) and USNS Supply (T-AOE-6) were involved in a collision while conducting a replenishment-at-sea in the Caribbean Sea. The incident, which took place during the afternoon, resulted in minor injuries to two personnel, who are currently reported to be in stable condition. Thankfully, both naval vessels were able to proceed with their missions following the collision, which is now under investigation. This mishap occurred shortly after the USS Truxtun set sail from Naval Station Norfolk, Virginia on February 3, 2026, marking the beginning of a scheduled deployment. The destroyer is manned by approximately 300 sailors and includes 26 personnel from the Helicopter Maritime Strike Squadron 50, also known as the 'Valkyries.' The USNS Supply sustained some hull damage, with breaches noted above the waterline on its port side, necessitating its return to Ponce, Puerto Rico, on February 16 for repairs. Prior to the collision, the supply vessel had just completed a replenishment-at-sea with the Ticonderoga-class cruiser USS Gettysburg on January 29 as part of ongoing Southern Command operations aimed at countering narcotics and maintaining regional security within the Caribbean. The USNS Supply departed Ponce shortly before the accident on February 11 after arriving two days earlier. The USS Truxtun was noted to have returned to port for unspecified equipment repairs following its return from the deployment, but it was seen underway again by February 6, ready for its mission.
