In a tragic incident on September 11, 2022, a 28-year-old crew member aboard the cargo ship 'Weserborg' lost his life during loading operations in Panama City. The accident occurred when he was crushed under heavy cargo while three workers were maneuvering five substantial rolls of paper, each approximately seven feet tall, into the vessel’s cargo hold using a crane. Investigations have revealed that the crane operator lacked visibility of the signalman directing the loading process, a critical oversight by Premier Bulk Stevedoring LLC, the marine cargo contractor from Mobile responsible for the operation. Following the incident, the Occupational Safety and Health Administration (OSHA) conducted an examination and identified serious safety violations within the company's procedures. Notably, it was determined that the supervisors in charge of cargo handling had not completed essential accident prevention training. The Department of Labor (DOL) emphasized on March 10, 2023, that the worker's death could have been avoided had Premier Bulk Stevedoring adhered to established federal safety guidelines. OSHA's investigation cited the company for two serious safety violations, in addition to a repeat violation related to unsafe loading practices. This is not the first time Premier Bulk Stevedoring has faced scrutiny; they were previously cited less than two years ago for similar unsafe loading operations. Following the recent incident, Premier Bulk Stevedoring LLC has a 15-day window to either comply with the citations, make payment of the proposed fine of $43,750, request an informal conference with OSHA, or contest the findings. The ongoing safety concerns surrounding their operations continue to raise alarm within the maritime industry.
