Tragic Incident Aboard 'Coresky OL': Able Seaman Fatally Injured Due to Cable Snap

In a devastating accident on April 3, 2019, an able seaman aboard the cargo vessel 'Coresky OL' lost his life when a cable snapped due to excessive tension during loading operations at Eastland Port in Gisborne. This tragic event was detailed in a report from the Transport Accident Investigation Commission, officially released on June 25, 2020. The incident occurred as the crew was utilizing a crane to apply tension to a wire rope for securing a load of logs onto the ship's deck. The wire rope was arranged in a complex zig-zag pattern, resembling a bootlace, and was threaded through multiple pulleys supported by foot wires along the cargo. As two able seamen monitored the tension, a foot wire broke after the crane's heaving concluded, leading to a dangerous recoil of the securing equipment. This recoil resulted in a fatal injury to one of the able seamen, compounded by the high load applied by the crane and the specific configuration of pulleys used. The Commission noted that the crew lacked adequate knowledge regarding the risks presented by wires under tension. Furthermore, the cargo securing manual provided insufficient guidelines to establish a safe operational procedure for cargo securing tasks. In light of these findings, Taiwan-based operator Shih Wei Navigation Co. Ltd. has been strongly advised to conduct a thorough safety assessment for vessels engaged in transporting deck log cargo to prevent future tragedies.

In a heartbreaking work-related accident, a 50-year-old crew member aboard the bulk carrier 'Coresky Ol' tragically lost his life after being struck by a loose wire rope while securing timber on deck. The incident occurred early in the morning on April 3, 2019, in Gisborne, shortly before 2 a.m. The crew member sustained serious injuries when the wire rope snapped during the lashing process of the logs intended for shipment. Despite the rapid response of emergency services, who arrived within minutes, the crew member succumbed to his injuries later in Gisborne Hospital. The 'Coresky Ol' had just finished loading logs the previous evening, around 9 p.m., and was preparing to depart for Incheon. Following the accident, authorities detained the vessel to conduct an investigation, and it remained in port on April 3.
