Tanker 'Orakai' Experience Engine Troubles Near Cartagena

On the morning of September 13, 2016, the tanker 'Orakai' experienced an engine malfunction approximately 13 miles south of Cartagena, while navigating the Alboran Sea. The vessel was carrying 4,732 tons of ethanol and was bound for Fos Sur Mer, with a crew of 12 members on board. Upon realizing the situation, the ship requested support, prompting the dispatch of the Spanish salvage tug 'Clara Campoamor' (IMO: 9338022) for assistance. Due to adverse weather conditions, the towage operation extended over a period of 24 hours. MAIB Releases Collision Investigation Report Involving 'Orakai' and 'Margriet'

The UK Marine Accident Investigation Branch (MAIB) has published its comprehensive investigation report detailing the collision between the chemical tanker 'Orakai' and the beam trawler 'Margriet' that occurred in the North Sea on December 21, 2014, under severe gale conditions. At the time of the incident, the 'Orakai' was navigating on autopilot at a speed of 10.5 knots on a course of 220°, while the 'Margriet' was traveling ahead at a reduced speed of five knots on a parallel course. The officer of the watch aboard the tanker left the bridge for a brief errand, instructing the ordinary seaman on lookout duty to notify him when the trawler was within a mile. Unfortunately, the officer on the 'Margriet' was unaware of the tanker’s proximity and proceeded to alter his course, setting the stage for a potential collision. Despite the lookout’s attempts to alert the officer on the 'Orakai', the call went unnoticed. The vessels collided, resulting in significant damage to the 'Margriet' and a spill of approximately eight tons of diesel fuel, while the 'Orakai' sustained only minor damages. The MAIB's investigation revealed that the lookout in the 'Margriet’s' wheelhouse failed to maintain an effective watch, as he did not spot the 'Orakai' until it was just one nautical mile away, three minutes before altering course. Furthermore, it was noted that the officer of the watch on the 'Orakai' had vacated the bridge 22 minutes prior to the incident, leaving the lookout unable to communicate effectively. It is believed that had the officer remained on the bridge, the collision might have been avoided. In the aftermath of the incident, South End Tanker Management B.V., which manages the 'Orakai', has implemented measures to enhance the effectiveness of its safety management protocols and has informed crews about the seriousness of the incident. The MAIB has also issued recommendations to both South End Tanker Management B.V. and Kafish B.V., the ownership entity of the 'Margriet', with the aim of optimizing navigational watchkeeping standards aboard their vessels. Tanker Collides with Fishing Vessel Off Dutch Coast

On the morning of December 21, 2014, a significant maritime incident unfolded as the KNRM service in Hoek van Holland and Stellendam received an urgent call at 07:00 AM. The British fishing vessel 'Margriet - LT 36' (IMO: 9284166) was involved in a collision with the tanker 'Orakai', approximately 45 miles away from the Dutch shoreline.
