A harrowing incident occurred off the coast of Norfolk, resulting in the tragic death of sailor Ryane Palabrica, who was just 26 years old. On April 15, 2012, Palabrica, an ordinary seaman, entered a cargo tank of the Stolt Skua without the necessary authorization, exposing himself to a hazardous atmosphere that was dangerously low in oxygen. During a routine cleaning of pyrolosis gasoline tanks, the ship’s safety protocols were disregarded, as entry into the tank required both an air quality assessment and explicit permission from the chief officer aboard.
